Recording a destruction

Open Destructions and use Add a destruction record: the date, the NDC, the quantity, the method and the two names. Each record has a PDF to sign and keep.

The DEA's rule

Each destruction is recorded on DEA Form 41 with the names and signatures of the two employees who witnessed it. Form 41 is kept as a record, not filed online.

Florida's rule

Florida is stricter. Each destruction is recorded on DEA Form 41, kept as a record, not filed online. In Florida it is witnessed and signed by the PDM and a DEA agent or a Department of Health inspector, or by the PDM and a medical director, director of nursing or sworn law enforcement officer. Or the stock goes to a reverse distributor: Schedule II on a DEA Form 222 or an electronic order.

NarcCount's PDF

The PDF is NarcCount's record of the destruction, with a line for each name to sign. It is not the DEA Form 41 itself.

Sent away, not destroyed

Pick Sent to a reverse distributor or Returned to the supplier as the method. The PDF then says the stock was sent away, not destroyed at the pharmacy.
